Student Assessment & Interview

New students complete a brief academic assessment to help us place them in the right grade level.

Assessment takes approximately 45–60 minutes

Enrollment Confirmation & Payment

Upon acceptance, sign the agreement and pay the registration fee through our secure online portal.

Required Documents

Please have the following documents ready when you submit your application.

Completed enrollment application form
Copy of student birth certificate
Copy of student social security card
Parent or guardian photo ID
Two recent passport-size photos of student

Health & Medical

Up-to-date immunization records (Florida Form 680)
Physical examination report (within 12 months)
Health history form (provided by MCA)
Special medical needs documentation (if applicable)

📚

Academic Records

Previous school report card (last 2 years)
Standardized test scores (if available)
Transfer form from previous school (if applicable)
IEP or 504 plan (if applicable)

Signed Parent-Student Handbook agreement
Signed Code of Ethics and Conduct form
Registration fee payment ($200)

Registration Fee

2025–2026 Enrollment Year

One-time registration fee for all grade levels.
